During the open enrollment period, employees may elect to cover your children on your medical plan until your child reaches age 26, provided your child is not eligible for other employer-sponsored coverage.
To enroll a child currently not covered on our employee medical plan, employees must:
1) provide a copy of their child's birth certificate.
2) complete and sign an Affidavit of Eligibility for Children Under Age 26, and
3) complete a Health Insurance Enrollment Form.
All three items must be submitted to your HR Department by June 10, 2011.
Note: Those employees with dependent children over age 19 who have already submitted student certification for 2011 do not need to complete the enrollment process described above during this open enrollment period. Student coverage will continue through December 31, 2011. The Office of Employee Benefits will contact you in October 2011 to request the necessary documentation to continue coverage for your student dependents after December 31, 2011.
Dental and vision coverage will continue to be available only for dependent children up to age 19 and full-time students up to age 25.
